Some of the information requested covers, inoculation dates, microchip numbers, vet information etc. – if at any time you need to check other information, do please ‘Save for Later’ to avoid any inconvenience.

PLEASE NOTE, we CANNOT accept any dog without your pet’s PIF being current, and all vaccinations and treatments eg working/flea) must be up to date and evidenced with a certificate from your vet. If your pet’s inoculation or treatment dates have changed since your last booking with MyWaggyTails, please update and submit this form with your booking.
